The Self-assessment Wheel is a tool that can help you identify some of the main things you can do as part of your self-management plan.

The tool is divided into 8 sections: Food and water, Exercise, Well-being, Stress and smoking, Treatment, Blood sugar, Screening, and Heart health. Self-reflect on how true each statement is to your diabetes care and heart health, with 1 being strongly disagree and 5 being strongly agree.
